WHAT TO BRING TO YOUR EYE APPOINTMENT

1. Medical and vision insurance cards.

2. Government-issued picture ID.

3. Current glasses, sunglasses, contact lenses or a copy of your most recent glasses prescription (Rx).

4. A list of all current eye drops and/or oral medications you are using, including over-the-counter medications.

5. Contact information for your physician(s) and a list of any health problems you may currently be experiencing or have had in the past.

6. For your convenience, we have made patient forms available online so that you can print and fill them out at your convenience. When you come in for your visit, please bring the completed forms with you and provide them to the receptionist when you arrive. If you prefer, you may arrive for your appointment a few minutes early and complete the forms in our office.
